Participant Waiver, Release of Liability & Media Consent Hosted by Analog.

Acknowledgment of Risk

I voluntarily choose to participate in today's guided breathwork event hosted by Breathe With Freddy LLC d/b/a Analog ("Analog"). I understand breathwork may involve deep or rapid breathing, breath holds, and movement that may cause dizziness, tingling, numbness, muscle cramping, changes in heart rate, emotional release, discomfort, or, in rare cases, fainting. I may stop or return to normal breathing at any time.

Medical Acknowledgment

I represent that I am physically able to participate. I understand breathwork may not be appropriate for individuals who are pregnant or who have epilepsy, serious cardiovascular disease, uncontrolled high blood pressure, recent surgery, detached retina, glaucoma, aneurysm, severe psychiatric conditions, or other medical conditions that make intense breathing inadvisable. I accept responsibility for obtaining medical advice if needed.

Assumption of Risk & Release

I voluntarily assume all risks associated with participation. In consideration of being permitted to participate, I release and hold harmless Breathe With Freddy LLC d/b/a Analog, its owner, instructors, employees, contractors, volunteers, landlord, affiliates, event partners, sponsors, and agents from any claims, liabilities, injuries, damages, losses, or expenses arising from my participation, except to the extent caused by gross negligence or willful misconduct where prohibited by law.

Photography & Video Release

I understand photography, videography, and audio recording will occur during today's event. I grant Analog permission to use my image, likeness, voice, and appearance in promotional, advertising, marketing, website, social media, and other business materials without compensation.
